As of August 1, 2007, we will no longer be participating with QualCare, Core Source, GHI, MagnaCare, Multiplan, and Beech Street.

We do participate with the following insurance plans:

     Note:  we will accept siblings of current Amerigroup patients after   5/1/09.  You must notify us in advance so that we may include a sibling on the Amerigroup panel.      

Cigna                   

 PHCS   (however, not all plans-please call our business office in advance to be sure if we accept your particular plan)                             

If your Insurance Company requires a co-pay, you will be responsible to pay it at the time of the visit.  If you have any deductibles or co-insurance, you will be billed for the appropriate amount after we receive payment from the insurance company,  YOU MUST BRING YOUR INSURANCE CARD WITH YOU TO EACH VISIT.

We require that you notify us of any change in your insurance.  Please familiarize yourself with the benefits of your plan as well as the restrictions.  You are ultimately. responsible for services that are not covered by your plan.
